For your convenience, you can print these forms, complete them, and submit them to the Office of the Registrar in Academics. Forms also can be faxed to 215-591-5749.
| Change of Address or Phone Number | Use this form to change your address and/or phone number. |
| Change of Name or Social Security Number | Use this form to change your name or social security number. Please keep in mind that you will be asked to provide proof of the changes. |
| Enrollment Verification Request | This form is used to verify your status at DeVry. It can be used for health insurance or for other services that need you to provide proof that you are or were a student at DeVry. |
| Program Transfer Application | Use this form to get permission to transfer from one program to another. Please keep in mind that you must meet the requirements for the program you want to transfer to and be approved by the program deans. |
| Resumes - Transfers Enrollment Agreement Addendum | DeVry students who leave their program for one or two semesters or who transfer to another program or DeVry location must sign this addendum prior to resuming or transferring. Students who leave the program for 3 or more semesters will be asked to execute a new agreement prior to resuming. Students who have signed an enrollment agreement prior to a tuition increase sign this addendum at registration to acknowledge receipt of notification of the increase. |
| Transcript Request Application | Use this form to request an official or unofficial transcript. Please keep in mind that there is a $5.00 fee for each transcript. You may mail in this form with a $5.00 check or money order for each transcript requested, or you can process it on campus. Faxed requests are not accepted. |
| Location Transfer Request Form | Use this form to request a transfer from one DeVry location to another. Complete this form and the "Resumes-Transfers Enrollment Addendum Agreement" and return them to the Registrar's Office. Requests should be files by the end of the 10th week of the semester prior to the transfer. |
